How to Register

People who would like to register with us, who live within our boundary area, should come to the reception desk during surgery hours (preferably after 11.00am).

Please bring your NHS medical card, if you have it, as well as confirmation of identity (passport, birth certificate or photo-card driving licence) and proof of your residential address (e.g. recent utility bill, council tax bill, bank statement).

Patients are registered with the Practice, in the name of one of the Practice GPs, although you are free to consult any doctor, subject to appointment availability. New patients are invited to attend for a health check with one of the Practice nurses.

Disabled Access

We have full disabled access at the Albany Practice.
